About the role:

We are seeking an experienced and compassionate Registered Nurse to join our growing Home Care Services this rewarding role youll deliver high-quality clinical care that supports our clients to remain independent in their own homes while building meaningful relationships and contributing to a service that truly makes a difference.

Part-time - 45.6 hours per fortnight and days negotiable

Hourly pay rate - $47.62 Super. Salary packaging

Role Specific Requirements:

  • Undertaking clinical assessments of home care clients
  • Providing excellent care and service to clients in line with their personalised care plans
  • Monitoring chronic diseases and palliative conditions
  • Wound care continence care palliative care medication management and management of chronic diseases
  • Achieving the optimum care for clients by being responsive to their changing needs
  • Liaising with and referring to other organisations as required to provide for the clients needs
  • Assist the Clinical Lead with other duties as required

Qualification and Experience:

Essential

  • Bachelor of Nursing and registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Basic IT and smart phone skills/literacy
  • Ability to exercise initiative and discretion
  • Ability to maintain a high level of confidentiality
  • Excellent communication and interpersonal skills including writing reports and other associated documentation
  • Ability to work with individuals from a range of social financial and cultural backgrounds
  • Ability to cope with changes in work demands to think outside the box and problem solve
  • NDIS Worker Screening Check
  • Australian work rights / appropriate visa
  • Current National Police Clearance (not older than 6 months)
  • WA Drivers Licence

Desirable

  • Dementia training or experience in dementia care.
  • Previous experience in community nursing
