Summary: Under the supervision of the Extended Care Services Director the SAS & AUSAA Coordination Specialist is responsible for ensuring seamless coordination of services for clients supported through Social Adjustment Services (SAS)
- Additional Ukraine Supplemental Appropriations Act (AUSAA) program. This role provides case management and program coordination to support Ukrainian populations and other displaced individuals arriving in the United States due to the conflict in Ukraine. The Specialist ensures eligible clients receive holistic culturally responsive and trauma-informed services that address immediate needs support integration and promote long-term stability and self-sufficiency.
Position Responsibilities:
* Service Coordination Oversee and align services across SAS and AUSAA to ensure clients receive comprehensive non-duplicative and timely support tailored to their unique needs.
* Case Management Provide direct services and referrals to AUSAA-eligible clients assisting with housing employment education healthcare legal services and other essential resources.
* Enrollment & Eligibility Support client enrollment in AUSAA case management ensuring accurate eligibility determinations and timely access to program benefits.
* Documentation & Reporting Maintain complete and accurate client records including intakes assessments and service plans. Enter and track data in internal databases and prepare reports as required by program guidelines.
* Client Empowerment Guide clients in navigating U.S. systems and accessing resources fostering self- sufficiency and community integration through education and advocacy.
