Job Summary:
This role is for a Home Health Physical Therapist responsible for providing physical therapy services to patients in their homes. The therapist will manage a caseload of approximately 4-5 patients per day focusing on improving patient mobility function and overall well-being. The position requires timely communication thorough documentation and the ability to work independently while adhering to productivity standards.

Responsibilities:
- Evaluate patients physical condition including their medical history functional abilities and limitations.
- Develop and implement individualized treatment plans to address patient needs and goals.
- Provide skilled physical therapy interventions such as therapeutic exercises manual therapy and gait training.
- Monitor patient progress and adjust treatment plans as necessary.
- Educate patients and their families on proper techniques for self-management and injury prevention.
- Document patient care accurately and completely in accordance with professional standards and regulatory requirements.
- Communicate effectively with physicians other healthcare professionals and patients families.
- Manage a caseload of approximately 4-5 patients per day potentially visiting up to 6 patients while meeting productivity standards.
Required Skills & Certifications:
- Active Physical Therapist license in WA or compact license.
- BLS certification.
- Minimum of 1 year of recent skilled Physical Therapy Home Health experience (within the last 3 years).
- Expert-level OASIS documentation skills.
- Reliable transportation and valid drivers license.
Preferred Skills & Certifications:
- Not specified.
Special Considerations:
- Traveler must have their own transportation.
- Occasional maximum radius of 100 miles due to territory.
- No weekend or on-call requirements.
Scheduling:
- 8-hour day shifts.
