Are you a Physical Therapist Assistant who loves coaching encouraging and building relationships with patients Do you believe that consistency between clinic visits is key to patient success Aquacare & Fitness Forum Physical Therapy is expanding our Remote Therapeutic Monitoring (RTM) program and we are looking for a PTA with great communication skills confidence and compassion to serve as a Care Navigator within our patient support team. Experience with Remote Therapeutic Monitoring is required!

What This Role Involves
As part of our RTM Team you will support patients in-between their clinic visits to increase home program consistency and overall outcomes. You will work closely with the evaluating therapist through our RTM platform (Limber) to:
- Monitor patient progress remotely
- Conduct phone/video follow-up check-ins
- Provide encouragement education and accountability
- Document meaningful communication within the RTM software
- Identify barriers and communicate updates to the treating therapist
- Help patients stay engaged in their plan of care
This position is ideal for a PTA who:
- Enjoys patient interaction and relationship-building
- Communicates clearly and confidently
- Is organized proactive and motivated to see patients succeed
- Values a role that blends clinical knowledge with coaching and support
- Flexible schedulingwork from home or hybrid
- Can be combined with in-clinic hours if desired
- Training is fully provided (no prior RTM experience required)
- Licensed PTA (Maryland or Delaware depending on location)
- Strong communication and follow-through skills
