Job Overview:

We are seeking an experienced Clinical Pharmacist to support primary care services remotely. This is a fantastic opportunity for a GPhC-registered pharmacist with a background in GP practices and structured medication reviews to work flexibly from home.

Key Responsibilities:

Conduct structured medication reviews for patients in primary care.
Provide clinical support and advice to GP practices.
Ensure safe effective and evidence-based use of medicines.
Maintain accurate patient records using SystemOne.
Collaborate with multidisciplinary teams to optimise patient outcomes.

Qualifications and Experience:

GPhC Registration as a Pharmacist.
Primary Care / GP Practice experience is essential.
Experience conducting structured medication reviews.
SystemOne IT system experience is essential.
Excellent communication and clinical decision-making skills.
