The Registered Nurse Case Manager will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills with patients providers and team members are essential.

Location:Apple Valley California United States
Responsibilities:
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide skilled nursing care.
- Manage patient cases.
- Provide hospice care.

Required Skills & Certifications:
- Clinical Assessment skills

- Minimum of two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work

Special Considerations:
- Must work a minimum of 4 weekend shifts in a 6 week period.
