Hiring: Travel RN - Case Manager
Location: Glenwood Springs CO 81601
Start Date: 11/17/2025
Shift: Days 36/48hr/wk
Weekly Gross: $3400/week $3600/week
Duties:
- Develop and implement individualised care plans for patients across various settings including assisted living nursing homes and home care.
- Coordinate medical services medication administration and therapy sessions in accordance with healthcare protocols.
- Monitor patient progress adjust care strategies as needed and communicate effectively with patients families and healthcare teams.
- Supervise and manage nursing staff and ancillary personnel to ensure high standards of patient care.
- Assist with specialised care such as Alzheimers disease dementia hospice support and working with people with disabilities.
- Conduct assessments related to anatomy knowledge laboratory results medical imaging and other diagnostic procedures.
- Provide education on health management topics including medication usage anatomy basics and chronic disease management.
- Ensure compliance with healthcare regulations and maintain accurate documentation of all patient interactions and treatments.
- Support patients requiring toddler or specialised eldercare such as dementia or Alzheimers care by applying tailored nursing approaches.
Reuirements:
- Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
- State licensure as a Registered Nurse (RN).
- Minimum of 3 years RN Case Management Experience is required.
