Hospice of Marion County A Member of Empath Health
Schedule: Monday Friday Daytime Hours
Hospice of Marion County part of the Empath Health family is seeking an experienced and compassionate Field RN/Case Manager to join our team. This position provides direct hands on nursing care and case management for patients in East Marion County helping to ensure comfort dignity and exceptional end-of-life care.
Key Responsibilities
Identify and coordinate patient and family care needs providing preventative and supportive care for terminally ill patients.
Utilize strong assessment intervention and teaching skills to maximize comfort and quality of life.
Collaborate with interdisciplinary team members to deliver comprehensive and individualized care plans.
Maintain accurate timely and compliant documentation in accordance with Empath Health standards and hospice regulations.
Serve as a trusted resource and advocate for patients and families throughout their hospice journey.
Promote and uphold Empath Healths mission vision and values in all aspects of care delivery.
Position Requirements
Education: Associates degree (A.A.) or equivalent from a two-year college or technical school.
Minimum of one (1) year of related experience and/or training; equivalent combination of education and experience considered.
Licensure:
Valid Florida Registered Nurse (RN) license.
Valid Florida Drivers License and reliable transportation.
Experience: One (1) year of experience in an acute or post-acute care setting preferred.
