Program Description: We are seeking a Case Manager QMHA to play a pivotal role in our Aid and Assist restoration program. This position is part of a two person team supporting individuals under the Aid and Assist population. This position will provide highly individualized and intensive recovery-oriented services to achieve specific goals and increase resiliency for individuals identified with a serious mental illness which resulted commission of a crime (Aid and Assist) as a result of their mental illness.
The Case Manager QMHA working with this program plays a vital role in supporting individuals transitioning from the Oregon State Hospital into the community. This position ensures continuity of care by initiating contact while clients are still hospitalized coordinating essential services such as housing and medication and providing ongoing support post-discharge. The Case Manager serves as a bridge between institutional care and community integration promoting stability recovery and independence.
Position Description: The Case Manager QMHA is responsible for assisting clients access a variety of services and supports. Additionally this role serves as an advocate and works with clients to meet their goals and needs. This position must understand appreciate and respect the diversity and cultural differences within our Cascadia community. As such it is expected that this position promotes integrated care our vision of trauma-informed and person-first approaches and helps create a work environment of inclusion safety and acceptance.
Essential Responsibilities

- Provide mental health case management and skills training with clients as indicated in their treatment plan and as directed by the treatment team.
- Work with the treatment team on assessing treatment needs support development and monitoring provide consultation to mental health aides and transition planning -including completion of referrals attending screenings advocacy maintaining housing obtaining resources benefits and consultation regarding client needs.
- Provide individual and group: skills training CPST activity therapy and meet productivity expectation.
- Effectively engage clients to participate in treatment services.
- Integrate best practices into the day-to-day operations including: Person-Centered Approach Harm Reduction Trauma Informed Care Motivational Interviewing Strength-Based Approach and Recovery Model.
- Create and maintain cooperative relationships with outside providers in securing benefits advocating for clients and connecting to acute care providers.
- Coordinate treatment and continuity of care with community stakeholders guardians representatives from the legal system hospital care medical care other community-based agencies and other Cascadia team members to maintain treatment team communication.
- Attend and participate in clinical staff meetings to provide input and gain knowledge and communicate program and client needs.
- Provide monitoring and supervision to support clients in developing and maintaining structured activities in alignment with their personal goals and any required conditions for community integration.

Regulatory & Compliance:
- Comply with laws regulations policies and procedures under which the program and company operates.
- Assist in crisis situations as appropriate following protocols and procedures in coordination with the treatment team.
- Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
- Maintain accurate and up-to-date documentation that ensures compliance with all agency policies and procedures and local state and federal regulations and billing requirements.
- Understand safety regulations and evacuation procedures; participate in regular safety drills in compliance with program policies and procedures.
- Complete annual employee training in compliance with program and company requirements.
- Provide guidance support and training to other staff in alignment with company mission and values.
- Participate in all scheduled staff meetings supervision sessions and other departmental and company meetings.
Qualifications
Education:
- A bachelors degree in a behavioral science field from an accredited college or university or three years of fulltime professional experience providing direct service to individuals in a social service setting is required.
- Must demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA) as outlined in the Oregon Administrative Rulings (OARs). Certification must be kept current and in compliance with OARs through MHACBO.
Experience:
- A minimum of 1 year experience working with adults with severe and persistent mental illness is required.
Specialized Knowledge Skills and Abilities:
- Effective communication and active listening skills.
- Effective interpersonal and customer service skills.
- Ability to work proactively and effectively in a team and independently.
- Knowledge of Microsoft Office suite.
- Operate computer hardware and software at a level needed to effectively perform job functions.
Other:
- Driving is required an employee must possess a valid drivers license a minimum of one year driving history ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
- All client-facing positions must have the ability to perform Basic Life Support (BLS) and properly use AED equipment. BLS certification training will be provided within the first 60 days of hire. Certification must remain current.
Working Conditions
Environmental: Work is performed in one or more of the following: clinical office acute care settings and community setting including client residences correctional facilities shelters hospitals public places and other social service sites. Meal periods and breaks are subject to interruption. Much of the work is performed during regular business hours with the potential of extending into after hours only for crisis response. Universal Precautions and remaining alert to the environment are critical to address the potential for violent behavior exposure to disease biohazards noise and contaminants.
Mental: The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management addition this position requires exercising sound judgment. Decision-making may occur in a moderate to high stress situation where a client may be experiencing a crisis.
