The Patient Safety Officer for the Patient Safety Organization serves in a role that models and promotes a culture of psychological safety where learning from errors and fair accountability are considered norms. The role leads and directs a model which supports reductions of harms readmissions as well as promoting the National Quality Program (NQP) initiatives

COMPANY DESCRIPTION

Frye Regional Medical Center located in the beautiful foothills of North Carolina is a 355-bed acute care hospital 81-bed Behavioral Health facility and more than 70 primary and specialty care providers.

Essential Functions

  • Working with all services to hardwire reliable delivery of healthcare systems that incorporate best practice patient safety methodologies.
  • Initiating and overseeing the development of a comprehensive patient safety improvement program inclusive of the analysis and trending of data related to hospital initiatives to reduce or minimize the risk of health care/medical errors to patients.
  • Providing proactive and reactive safety initiatives including participation in root cause analysis failure mode effect analysis and Sentinel Event Alerts. This information will be used to develop a process and plan for change that can be constantly monitored and evaluated for its effectiveness.
  • Providing leadership and/or participating in patient safety committee and communicating vertically and horizontally within the organization.
  • Providing needed direction to ensure that clinical services are provided in accordance with safety standards established by federal and state regulatory agencies and the Joint Commission accreditation standard including the National Patient Safety Goals.
  • Developing patient safety score card and/or dashboard indicators in conjunction with the medical and nursing staff and the hospital leadership as a mechanism for tracking adherence to agree upon patient safety goals.
  • Participating in the executive patient safety rounding program and developing clear expectations and recommending for analyzing trends identifying root-causes and ultimately developing solutions to issues identified during the rounding process.
  • Works in partnership with senior leadership medical staff leadership Director of Quality and in collaboration with Duke LifePoint PSO.
  • Assisting in and/or coordinating data collection aggregation analysis and process/system improvements related to national patient safety goals in partnership with quality staff.
  • Facilitating proactive risk assessments including failure mode effect analysis gap analysis of published Joint Commission Sentinel Event Alerts.
  • Be a driver and owner of educating and implementing Lifepoint Healths Foundational 5.
  • Other duties as assigned.

Job Requirements

Qualifications

Minimum Education

Bachelors Degree required BSN preferred

Masters Degree preferred.

Minimum Work Experience

Minimum 5 years RN experience

Minimum 2 years management experience preferred.

Requires critical thinking skills decisive judgment and the ability to work with minimal supervision.

Must be able to work in a stressful environment and take appropriate action.

Current RN license to practice in NC (NC licensure or multi-state (compact) license).

Basic Life Support (BLS)Certification.

Professional Patient Safety (CPPS) or completion within two (2) years of hire. This certification must be maintained throughout employment.

Working knowledge of Lean Six Sigma preferred.
