Hospice Registered Nurse (RN) Case Manager
Muskegon to Grand Rapids
Full-Time MondayFriday 8a5p $5000 Sign-On Bonus!

What Youll Do
Perform thorough patient assessments and develop individualized care plans
Coordinate with an interdisciplinary team to deliver high-quality hospice care
Educate and support patients families and caregivers on hospice philosophy pain control and symptom management
Monitor changes in patient condition and communicate updates promptly
Provide emotional and spiritual support during end-of-life care
Serve patients in home settings nursing facilities and assisted living communities
What Youll Need
Graduate of an accredited nursing program (Diploma ADN or BSN)
Current RN license in the state of practice
1 year of clinical experience (hospice or home health preferred)
Reliable transportation valid drivers license and proof of auto insurance
Ability to lift move and assist patients (up to 50100 lbs)
