How you will contribute

A Clinic Case Manager who excels in this role:

• Identifies the targeted population and implements clinical interventions and protocols.
• Collaborates with PCP patients and members of health care team including continuum of care settings and community resources.
• Provides self-management support and includes patient/family as active members in the team approach.
• Facilitates close monitoring of high-risk patients to prevent and/or intervene early during acute exacerbations.

• Coordinates care between PCP and acute care hospitals specialists and post -acute care services.

What we are looking for

Applicants should have a state of Michigan RN license and possess an associate degree from an accredited nursing school (bachelors degree preferred).

Additional requirements include:

• Basic Life SupportHealth Care Provider (BLS-HCP)

• Two (2) years experience in adult medicine and pediatric patients.
