About the role
We are advertising for a full time Case Manager to join our team in the Hunter. Our office is based in Warabrook but we have clients all throughout Newcastle Lake Macquarie Maitland Cessnock and parts of the Upper Hunter. We are also advertising for a part-time role (4 days per week) - please indicate your preference when applying. Case Management experience is preferred and Diploma of Community Services or Cert IV is essential
Your main duties will include:
  • Assessing client needs developing a collaborative strengths-based Care Plan with the client and their carer/representative and implementing services which will meet their individual needs
  • Establishing the Care Plan within an allocated budget monitoring the budget and ensuring services adhere to the total budget
  • Facilitating referrals and work collaboratively with Nursing Allied Health services (internal and external) and Medical Practitioners to address identified clinical needs.
About you
  • Relevant tertiary qualification in the community sector
  • Demonstrated experience in the provision of case management services
  • Drivers Licence
  • Highly developed communication skills
  • Confidence using computer-based systems
