Job Summary

Position Summary

The Certified PCA/HHA provides compassionate personcentered care to clients in their homes under the supervision of a Registered Nurse and according to an individualized Plan of Care. The aide supports Activities of Daily Living (ADLs) selected Instrumental Activities of Daily Living (IADLs) and other authorized tasks to promote safety dignity and independence in accordance with New York State Department of Health (NYSDOH) regulations and agency policies.

Key Responsibilities

  • Personal care/ADLs: Bathing grooming oral hygiene toileting pericare dressing skin care to intact skin turning/positioning transfers and ambulation with approved assistive devices.

  • Nutrition: Consistent meal planning and preparation according to dietary needs; assistance with feeding as indicated; encouragement of fluids and nutrition as outlined in the care plan.

  • Mobility & safety: Fallprevention measures safe use of mobility aids home safety checks standard precautions and infection control.
  • IADLs (when authorized): Light housekeeping related to the client laundry bedmaking shopping/errands and accompanying to medical appointments when permitted.

  • Observations & reporting: Measure/record temperature pulse respiration and weight as directed; recognize and promptly report changes in condition incidents or hazards.

  • Documentation: Accurate timely electronic or paper visit notes timesheets and task checklists per agency standards.
  • Professionalism: Maintain client confidentiality cultural sensitivity and professional boundaries; uphold clients rights and dignity at all times.

Minimum Qualifications (Required)

  • Certification:

    • PCA: Completion of a NYSDOHapproved Personal Care Aide training program and valid PCA certificate.

    • HHA: Completion of a NYSDOHapproved Home Health Aide training or equivalency program and active listing on the New York State Home Care Aide Registry (HCAR); possess a valid HHA certificate.

  • Age & work authorization: At least 18 years old; legally authorized to work in the U.S.

  • Transportation: Reliable means to travel to client homes as assigned; ability to work in varied home environments.
  • Communication: Ability to read write and speak English sufficiently to follow the care plan and document services; additional languages a plus
