Description
The Case Management Engagement Specialist is responsible for engaging individuals with serious and persistent mental illness who are disconnected from traditional services or at risk of disengagement. The specialist builds trust with clients and their families assesses needs creates treatment plans and connects them to appropriate mental health medical housing and social services. They may also collaborate with schools and DCYF. This role focuses on outreach early intervention and reducing barriers to care with a strong emphasis on person-centered approaches. The ability to multitask manage time well and collaborate with other staff is required for this position.
Requirements
- Bachelors Degree in psychology social work psychosocial rehabilitation or related human services field OR Associates degree in related field and two years experience working with individuals with mental illness.
- Consistent positive employment history.
- Strong EMR abilities and willingness to complete paperwork in a timely manner.
- Understanding of mental illness and desire to reduce stigma and effectively support this population.
