PLEASE VISIT: Administration for Childrens Services (ACS) protects and promotes the safety and well-being of children and families through child welfare and juvenile justice services and community supports. ACS manages community-based supports and foster care services and provides subsidized child care vouchers. ACS child protection staff respond to allegations of child juvenile justice ACS oversees detention placement and programs for youth in the community.

The Division of Youth and Family Justice (DYFJ) provides a wide range of services and programs for youth at every stage of the juvenile justice process. Our goals are to (a) build stronger and safer communities and advance public safety by preventing juveniles from entering the New York Citys juvenile justice system (b) provide therapeutic treatment and services to youth in our custodial care and (c) help juveniles access the tools they need to leave the justice system for good. We strive to help families and young people by providing supportive services that are responsive to the needs of individual youth and families. Close to Home (CTH) is a residential placement and aftercare program for New York City youth who are adjudicated delinquent by the NYC Family Court and ordered into conjunction with the contracted providers ACS provides intake case management conferencing school support and monitoring services.

The Division of Youth and Family Justice (DYFJ) is seeking an outstanding Child and Family Specialist to serve as a Qualified Individual Specialist. The federal Family First Prevention Services Act (FFPSA) requires jurisdictions use the least restrictive form of foster care placement that is appropriate to meet the childs needs. For Qualified Residential Treatment Program (QRTP) placements law requires that the state-specified assessment must be completed by a Qualified Individual. CFS in the Qualified Individual functional assignment will complete all such assessments in NYC.

The Qualified Individual Specialist will be responsible for performing the following duties but not limited to:

- Convene a permanency team consisting of all appropriate biological family members relatives and fictive kin of the child as well as as appropriate professionals who are a resource to the family of the child. For children 14 years or older the family and permanency team must include members of the permanency planning team that the child selects in accordance with Title IV-E requirements.

- To make the level of care assessment consult the family and permanency team as to the appropriateness of the assessed level of care. Assesses the level of care utilizing an OCFS-approved age-appropriate evidence-based validated functional assessment instrument.

- Prepare a court report and written summary detailing the assessment findings. Provides such assessment determination and documentation to the court the parent or guardian of the child and to the attorney for the child and the attorney for the parent within federally determined timeframes.

- Attend court hearings to present and provide evidence of the assessment determination and documentation of the childs level of care. For children who remain in Qualified Residential Treatment programs as required assist the permanency planning team with obtaining documentation for the permanency hearings and re-assessments of the appropriateness of continued stay in the residential setting.

- Facilitate re-assessment for replacements if the court disapproves of the previously assessed QRTP level of care and collaborates with the placement office to plan for an alternate setting in alignment with the schedule defined by the court.

- Maintain complete and accurate records including extensive case record documentation in Connections or any other designated system of record in a timely manner.

Qualifications :

A Masters Degree in Social Work from an accredited school of social work and
1. A valid New York State Registration as a Licensed Clinical Social Worker (LCSW) and at least one year of full-time satisfactory post-graduate experience that includes substantial experience in one or more of the following modalities of practice: facilitation of family team conferences or family group decision-making meetings utilizing a conferencing model which emphasizes family engagement and consensus based decision-making; clinical group work with children youth parents or individual families; or the conduct of professional training on topics related to child and family services; or

2. A valid New York State Registration as a Licensed Master of Social Work (LMSW) and at least three years of the experience required in 1 above. Up to two years of this experience may have been in the supervision of social work practice areas described in 1 above; or

3. At least three years of the experience required in 1 above. A valid New York State Registration as a Licensed Clinical Social Worker (LCSW) or Licensed Master of Social Work license (LMSW) must be obtained within one year of appointment.

Note: Employees who fail to obtain their LCSW or LMSW within one year after appointment may have their probationary periods extended for no more than six months. Failure to obtain the LCSW or LMSW by the end of the probationary period will result in dismissal.

Note: At least one year of the required experience described in 1 2 and 3 above must have been obtained in the last five years.
