This position provides compassionate hospice care and patient support throughout New Kensington and nearby areas. The role involves managing and delivering patient care plans maintaining timely documentation and ensuring quality comfort-focused care for patients and families.

Key responsibilities

Coordinate and deliver individualized hospice care plans
Support patients and families within the assigned area
Complete documentation and charting accurately and on schedule
Manage time and workflow independently
Utilize tablets mobile devices and EMR systems effectively

Schedule

Shift begins 8:00 a.m. Saturday through 8:00 a.m. Monday
Scheduled rest periods provided during the shift

Travel

Local travel required within the assigned region

Qualifications
Required

Associate degree or higher in Nursing
At least 2 years of hospice experience
At least 1 year of experience with EMR systems such as TruChart EHR or similar
Active Registered Nurse (RN) license in Pennsylvania
Valid drivers license reliable transportation and current auto insurance

Preferred

RN license number provided for credential verification
