Position Description:
Inpatient surgery pharmacist position at Mayo Clinic Hospital- Saint Marys Campus in Rochester this role the pharmacist provides exceptional care to patients throughout their peri-operative journey; including pre-operative intra-operative and post-operative phases of medication management. Decentral clinical services consist of collaborative practice with multidisciplinary teams identification of drug-related problems provision and documentation of recommendations through progress notes regarding optimization of drug therapy admission and discharge medication reconciliation transition of care planning participation on discharge rounds intermittent surgical team rounds and direct patient care.
Job Responsibilitiesinclude assessment of medication therapy regimens medication therapy management medication order verification/entry medication distribution medication therapy monitoring and consultation documentation of medication therapy plans and medication use education and pharmaceutical care of Mayo Clinic pharmacy and other disciplines learners is expected.
Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. Opportunity for academic appointment at hiring and progression through appointment ranks through achievement.
Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience.
Additional Preferred Qualifications:
Completion of pharmacy residency (PGY1 or PGY2) and/or equivalent experience of 3 years in practice
Evidence of ongoing scholarly pursuits including research/quality improvement publications or scientific presentations
Relevant applicable pharmacy experience working in a progressive and collaborative practice environment
Experience and/or interest in teaching and/or precepting
Board Certification in Pharmacotherapy Specialty (BCPS)
License or certification:
License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
