We are seeking a highly motivated and experienced Registered Nurse to join our team as a Patient Care Manager in the Home Health sector based in Roswell. This role is key in overseeing and ensuring the delivery of high-quality compassionate care to patients within their homes.

Qualifications:

Education: BSN degree preferred or equivalent clinical experience
Licensure: Current Registered Nurse (RN) license in the state of Georgia
Experience:
- Experience in home health care or a related field.
- Previous experience in a home care setting with two years of management or supervisory experience preferred.
Technical Skills: Proficiency in electronic medical records and healthcare technologies; knowledge of home health regulations and standards
Soft Skills: Excellent communication and interpersonal abilities; strong organizational and problem-solving skills; leadership and team management capabilities

Key Responsibilities:

Lead and supervise a dedicated team of healthcare professionals to deliver exceptional patient care.
Coordinate patient care services and develop individualized care plans.
Conduct regular patient assessments and monitor the effectiveness of care plans.
Ensure compliance with all healthcare regulations and standards.
Collaborate with multidisciplinary teams to optimize patient outcomes.
Provide education and support to patients and their families regarding health conditions and care plans.
Maintain accurate and timely documentation of patient care and progress.
