DescriptionThe Public Health Nurse is a Registered Professional Nurse who provides safe competent quality care based on a designated group of patients and significant others in the community health continuum of care.
Responsibilities- Assists in development implementation and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements.
- Assesses and evaluates patient needs for and responses to care rendered and applies sound nursing judgment in patient care management decisions.
- Provides direct continuous care (where applicable) and management for a caseload of 12 to 15 scheduled patients daily and up to 5 walk in patients through application of the nursing process and approved Department of Nursing and institutional standards.
- Administer vaccines for scheduled patients as well as those prescribed by the PCP in the clinic on a daily basis. These include but are not limited to:
- Influenza vaccine; Pneumonia vaccine; T-dap; Hepatitis A; Hepatitis B; MMR; Varicella; HPV; PPD Polio; Typhoid; Pain medication Vitamin B12 injections Contraceptive and Hormone injections.
- Perform bi-weekly blood pressure checks of patients with a diagnosis of Hypertension uncontrolled by medication or patients who need follow up after being prescribed a new medication.
- Conduct Coumadin Clinic in collaboration with the PCP for patients and adjust medication based on the patients INR
- Perform wound care cleaning and dressing wounds and providing patient education on how to change a dressing if there is a need.
- Triage: Assess and evaluate walk in patients based on their complaints and triage them to the appropriate level of care
- Communication: Manage telephone encounters for the PCP and call center and route them (based on severity of complaints) to the appropriate level of care.
- Collaborate with the physicians in Community Based Outreach Programs (including schools practice site specific) where families are educated about their health and given a chance to ask questions about the medical field.
- Community Based Health: Health and educational screening which includes Hypertension teaching and Blood Pressure checks performed on the parents Breast Self-Examination teaching Diabetic Teaching and Colon Cancer screening.
- Participate in monthly Mammogram screening with the Community Mammogram Van which comes to specific sites monthly including weekends.
- Coordinate care with Outside agencies such as VNSNY Lab Corp Quest Diagnostics Physicians Offices other specialty clinics and Pharmacies.
- Collaborate with the physicians to coordinate care for the Population Health Panel and measure compliance with Federal Mandated Metrics by closing required care gaps.
- Perform Weight Management Education for patients with elevated BMIs who are candidates for Bariatric Surgery.
- Coordinates patient care activities based on established priorities including teaching not limited to:
- Diabetic education; Congestive Heart Failure (CHF) education; New Onset Hypertension education; Venous Stasis Ulcer education; Weight Management education;
- Coordinates patient care activities including huddling counseling discharge planning and directs/delegates appropriately to members of the health care team.
- Communicates effectively and professionally with patients significant others and members of the health care team about the patients plan of care.
- Participates in performance improvement nursing quality nursing research activities and utilizes same in clinical practice.
- Participates in the design measurement and assessment of care related structures processes and outcomes to improve the quality of patient care.
- Participates in maintaining a safe therapeutic and consumer oriented environment.
- Collaborates with members of the health care team the patient and significant others to plan and implement the patients care.
- Coordinates transfer to Emergency Room Urgent Care or alternate care facility as needed.
- Conducts post admission discharge phone calls on selected patients.
- Participates in departmental and interdisciplinary committees which determine policy standards and clinical procedures that affect nursing/PA practice.
- Utilizes and participates in the development of Hospital Departmental and Care Center standards interdisciplinary clinical/pathways policies procedures and guidelines as well as regulatory agency standards as appropriate resources.
- Participates in the development and implementation of a continuous quality improvement program/processes.
- Treats co-workers patients and families with dignity and respect.
- Performs charge responsibilities as assigned.
Qualifications- Bachelor of Science with a major in Nursing.
- Associate Degree or Diploma in Nursing with current matriculation in a BSN program with graduation date within one year of hire will be considered by the Chief Nursing Officer.
- Must have relevant clinical competence in area of nursing practice; new graduates possess current knowledge of the nursing process and its application.
- Evidence of ability to maintain interpersonal relationships communicate appropriately to others work effectively to solve problems and confronts issues appropriately
- Registered Nurse license in New York State with current registration
