As an RN Case Manager you will play a critical role in making our patients final days weeks and months as comfortable as possible. Responsibilities:
- Conduct initial client assessments and continuously refine care plans.
- Provide hands-on nursing care and patient education.
- Maintain open lines of communication with patients families and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
Qualifications:
- Licensed as a Registered Nurse / RN in the state you work in.
- Proficiency with Electronic Medical Records (EMR) software.
- Excellent planning organizing and communication skills.
- Ability to solve problems autonomously ensuring responsibilities are met while maintaining high standards.
- Valid Drivers License and reliable vehicle with current insurance.
