We are looking for a Dietitian with experience in chronic/long term conditions who is interested in delivering the Piki te Ora service to whānau in Hauraki. Based in Thames the role is part time (24 to 32 hours per week/0.6-0.8 FTE) permanent role with travel across the Hauraki Rohe when required.
The Dietitian is responsible for working with whānau/clients to improve their health and wellbeing through nutritional support contributing to the reduction of health inequalities for Māori whānau. This includes one to one consults and community education sessions. The role requires working collaboratively with community and health care professionals.
To be successful in this role you will need:
- A current APC and experience in primary health care/general practice or chronic conditions services
- The ability to build relationships and communicate effectively (verbally and in writing) putting our patients and their whānau at ease
- To be highly organised and be able to manage your time and workload effectively - particularly when it gets busy
- To be flexible energetic pro-active with a positive can do attitude - ready to pitch in and help out when needed.
- To be passionate about improving health outcomes for Māori.
