Are you passionate about supporting those who have served our country NCSS is seeking a dedicated Care Coordinator to join our team and provide integrated person-centered support to military members veterans and their families.
In this role youll play a key part in ensuring individuals receive timely coordinated care that reflects their goals and needs. Youll collaborate across systems working closely with healthcare providers community agencies and the Department of Veterans Affairsto help clients access the right level of care navigate benefits and transition smoothly between services.
The ideal candidate thrives in a fast-paced team-oriented environment and values quality compassion and collaboration. Youll support ongoing case management efforts assist with administrative and documentation processes participate in quality improvement initiatives and maintain close communication with clients and partners to promote engagement and wellness.
Your work will focus on empowering veterans and service members by addressing unique needs such as trauma recovery reintegration challenges housing stability and behavioral health coordination. This position requires flexibility strong communication skills and a commitment to trauma-informed culturally competent care.

Qualifications
Bachelors Degree preferred; or equivalent combination of related education training and experience. Experience working with or within the VA Department of Defense or veteran-serving nonprofits is highly valued.
