POSITION SUMMARY:
A hospice outpatient case manager is a registered nurse responsible for overseeing the comprehensive care of terminally ill patients in their homes working closely with the patient their family and other members of the interdisciplinary team to develop and implement individualized care plans monitor symptoms provide emotional support and coordinate all aspects of end-of-life care while ensuring the patients comfort and dignity throughout the process.

MINIMUM QUALIFICATIONS:

Education:
Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing.

Required length of experience:

  • Previous experience in specific nursing area preferred.
  • Minimum of one (1) year experience as registered nurse in acute care/medical surgical area.
  • Prefer previous experience in hospice palliative care or oncology.
  • Case management skills including the ability to assess complex needs develop comprehensive care plans and coordinate care with multiple healthcare providers.
  • Excellent communication and interpersonal skills and ability to build rapport with patient and family members.
  • Self-starter with strong critical thinking and problem-solving skills.

Required licensure certification and/or registry:

  • Current license by the Ohio State Board of Nursing.
  • CPR certification required.
  • Must have a valid drivers license and current insurance coverage.
  • Prefer national certification in hospice and palliative care or oncology.
  • CHPN certification is preferred within two (2) years of employment.
