Essential Job Duties
- Provide care to clients in accordance with the individualized Plan of Care developed by the supervising nurse.
- Assist clients with transfers ambulation and prescribed exercises as instructed by the supervising nurse.
- Prepare modified diets as directed by the care plan.
- Observe and report changes in clients vital signs skin condition or symptoms to the supervising nurse.
- Remind clients to take prescribed medications as instructed.
- Perform light household tasks necessary for a safe and comfortable home environment such as cleaning and grocery shopping.
- Report any changes in the clients physical status promptly to the supervising nurse or agency.
- Submit required annual compliance documents (e.g. TB questionnaire).
- Complete 12 hours of mandatory annual compliance training as required by the New Jersey Board of Nursing.

Qualifications
- Active New Jersey Certified Home Health Aide (CHHA) license (required).
- Must be at least 18 years of age and able to read and write in English; bilingual candidates are encouraged to apply.
- Must have a physical examination meeting New Jersey State Department of Consumer Affairs requirements for home health care workers.
- Demonstrate compassion reliability and professionalism.
