Overview
We are seeking a compassionate and skilled Registered Nurse (Home Health) to join our clients dedicated team in Enfield Illinois. This role focuses on providing high-quality individualized nursing care to patients in their homes supporting recovery independence and overall well-being. The ideal candidate will bring strong clinical judgment excellent communication skills and a passion for patient-centered care.

Key Responsibilities
Perform comprehensive patient assessments and deliver skilled nursing care in home settings
Develop and implement individualized care plans in collaboration with physicians and interdisciplinary care teams
Educate patients and families on treatment plans medications and post-care instructions
Monitor and evaluate patient progress adjusting care plans as needed to achieve optimal outcomes
Maintain accurate timely clinical documentation in accordance with professional and regulatory standards
Coordinate with other healthcare professionals to ensure continuity and quality of care

Schedule and Shift Details
Monday through Friday standard daytime hours
Flexible scheduling with minimal weekend work
Occasional on-call or weekend assignments as required
This is a Pay Per Visit position

Travel
Local travel required within the Enfield area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association (assistance available if not current)
Valid drivers license reliable vehicle and current auto insurance

Preferred
Previous home health or community-based nursing experience
Strong communication critical thinking and organizational skills
