We are seeking a skilled and compassionate Registered Nurse (Home Health) to join our clients expanding team in Morton Grove this rewarding role you will provide high-quality personalized nursing care to patients in their homes-helping them maintain their independence comfort and quality of life. The ideal candidate will possess strong clinical skills exceptional attention to detail and a genuine commitment to patient-centered care.

Key Responsibilities
Conduct comprehensive patient assessments and provide skilled nursing care in home settings
Develop and implement individualized care plans in collaboration with physicians and interdisciplinary teams
Educate patients and families on medication management disease processes and post-care instructions
Monitor and evaluate patient progress adjusting care plans as needed
Maintain accurate timely clinical documentation in compliance with agency standards and state regulations
Coordinate with healthcare providers to ensure seamless continuity of care

Schedule and Shift Details
Monday through Friday daytime hours
On-call rotation: 1 weekend per month 2 3 on-call days per month and 2 holidays per year
This is a Pay Per Visit position

Travel
Local travel within the Morton Grove area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community-based care
Strong communication organization and critical thinking skills
