We are seeking a dedicated and experienced Registered Nurse (Home Health) to join our clients growing team in Mundelein Illinois. This position offers the opportunity to provide compassionate high-quality care to patients in the comfort of their own homes. The ideal candidate is a clinically skilled RN with excellent communication abilities and a strong commitment to patient-centered care.

Key Responsibilities
Deliver skilled nursing care and conduct comprehensive patient assessments in home settings
Develop implement and update individualized care plans based on patient needs and physician orders
Educate patients and families about medications treatments and disease management
Monitor patient progress documenting observations and interventions accurately and promptly
Collaborate effectively with physicians therapists and other healthcare professionals to ensure continuity of care
Maintain compliance with all agency policies regulations and professional standards

Schedule and Shift Details
Monday through Friday daytime hours
On-call rotation includes 1 weekend per month 1 2 on-call days per month and 2 holidays per year
This is a Pay Per Visit position

Travel
Local travel within the Mundelein area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Illinois
Minimum of 1 year of professional RN experience (home health experience preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable transportation and current auto insurance

Preferred
Strong clinical assessment and documentation skills
Ability to work independently and manage time efficiently
