Our client is seeking a compassionate Medical Social Worker to join their Home Health care team in Albion MI. This role involves providing emotional and resource support to patients in their homes while collaborating with the interdisciplinary care team to deliver exceptional patient outcomes. The ideal candidate is empathetic organized and committed to making a meaningful difference in patients lives.

Responsibilities include developing and implementing patient care plans supporting patients and families in Albion and surrounding areas maintaining accurate documentation managing caseloads efficiently and traveling locally as needed.

Schedule: Monday Friday daytime hours
Travel: Local travel within assigned area required (20% of time)

Required qualifications:

Masters degree in Social Work from an accredited program (CSWE-approved)
One to two years of post-graduate experience in a healthcare or home health setting
Active MSW license in the State of Michigan
Valid drivers license reliable transportation and auto insurance
Proficiency with EMR systems such as TruChart
Strong organizational and communication skills
