Our client is seeking a compassionate Medical Social Worker to join their Home Health team in Kansas City MO. The role involves supporting patients and families through emotional social and practical care coordination while collaborating with the interdisciplinary team to ensure quality in-home services.

Responsibilities include developing and implementing individualized patient care plans providing support in Kansas City and surrounding areas maintaining timely documentation and managing a flexible caseload with some local travel.

Schedule: Flexible part-time hours Monday Friday
Travel: Local travel required (20% of time)

Required qualifications:

Masters degree in Social Work from an accredited program (CSWE-approved)
One to two years of post-graduate experience in a healthcare or home health setting
Active MSW license in the State of Missouri
Valid drivers license reliable transportation and auto insurance
Proficiency with EMR systems such as TruChart
Strong organizational and communication skills
