Our client is seeking a compassionate and skilled Occupational Therapist to join their Home Health team in Detroit this role youll provide in-home Occupational Therapy services to patients under the direction of the attending physician supporting their physical cognitive and functional independence. You will also collaborate with the interdisciplinary care team to deliver comprehensive therapy care plans and provide education to patients families and staff.
Key Responsibilities-
Perform Occupational Therapy assessments to determine patient needs and develop individualized treatment plans.

Establish short- and long-term therapy goals focused on improving independence and functional ability.

Instruct patients and family members on therapeutic exercises adaptive techniques and self-care strategies.

Provide training in the use of orthotic prosthetic and adaptive equipment; design and fabricate assistive devices when necessary.

Conduct ongoing evaluations to assess patient progress and revise care plans as needed.

Maintain accurate clinical documentation and submit reports in accordance with agency policy.

Communicate with physicians and care teams regarding patient progress and treatment updates.

Educate and support other care team members in therapy techniques and patient equipment usage.

Participate in staff meetings in-service training and professional development activities.

Full-time schedule; specific hours and travel expectations will be discussed during the interview process.

Local travel required within the service area.

Bachelors degree or higher in Occupational Therapy

1 years of relevant clinical experience

Valid Occupational Therapist Registered (OTR) license in Michigan (MI)

American Heart Association Basic Life Support (AHA BLS) certification (ACLS/PALS also accepted)

Valid drivers license and reliable transportation

Experience documenting care using EMR systems preferred
