Job OverviewWe are seeking one full-time Home Health Occupational Therapist to join our mission-driven team in Alameda this field-based role you will help patients regain independence in activities of daily living by delivering evidence-based OT interventions adaptive-equipment training and caregiver education. Youll self-schedule roughly 5-6 in-home visits per day document care in the EMR and collaborate closely with an interdisciplinary teamall while working in a supportive environment that values collaboration patient-centered care and professional growth. The position includes mileage reimbursement and offers a full suite of medical dental vision PTO and 401(k) benefits. Open to relocating candidates who already hold an active California OT license.
Responsibilities
Key Responsibilities Evaluate functional abilities in the home
Implement ADL-focused therapeutic activities (dressing bathing cooking)
Recommend/adapt equipment for safety & independence
Update care plans and document in EMR
QualificationsLicensing
Active OT Therapist license in practicing state.
EducationOT degree (OTD/MOT/BSOT)
CertificationsCurrent CPR certification.
TransportationReliable transportation with valid drivers license and auto insurance.
Ideal CandidateCompassionate therapistwith a patient-centered approach to care.
Collaborative team playercapable of working within an interdisciplinary team.
Detail-orientedwith strong documentation skills.
