Responsibilities
Conduct comprehensive patient evaluations in home settings and develop individualized PT treatment plans
Deliver therapeutic exercises manual therapy and functional mobility training to improve strength balance and independence
Educate patients and caregivers on safety homeexercise programs and longterm selfmanagement strategies
Coordinate care with nurses OTs social workers and physicians to ensure seamless interdisciplinary support
Document patient progress outcomes and plan updates in the electronic medical record within required timeframes
Maintain HIPAA compliance safety standards and equipment sanitation on every visit
Qualifications
Active California Physical Therapist license (Doctorate or Masters preferred)
Open to new grads
Current CPR/BLS certification
Reliable transportation with auto insurance and valid CA drivers license
Strong clinical reasoning communication and patienteducation skills
Ability to work independently manage a dynamic caseload and adapt to changing patient needs
Ideal Candidate
A selfmotivated licensed California PT with 1 year of homehealth experience; excels at independent decisionmaking patient education and EMR documentation; thrives in fastpaced travelintensive roles; and values interdisciplinary teamwork and continuous learning.
