Our client is seeking a compassionate and skilled Occupational Therapist to join their Home Health team in Franklin PA. As a vital part of an accredited nationally recognized leader in home health and hospice youll collaborate with an interdisciplinary care team to deliver exceptional patient care and help patients regain independence and quality of life at home.
Key Responsibilities
Perform Occupational Therapy assessments and develop individualized treatment plans focused on restoring physical and cognitive function and independence
Implement therapy programs as prescribed by the attending physician
Educate patients and caregivers on self-care techniques adaptive equipment and home exercise programs
Fabricate or adjust orthotic prosthetic and assistive devices as needed
Reassess progress and revise care plans as necessary
Maintain accurate clinical documentation and submit reports in compliance with agency standards
Communicate effectively with physicians and interdisciplinary team members
Participate in staff meetings in-service training and professional development activities
Schedule and Shift Details
Monday Friday 8:00am 5:00pm
Light full-time (4 days/week)
No weekends holidays or on-call requirements
Travel
Local travel to patient homes in Franklin Somerset and surrounding areas (20% of the day)
Required Qualifications
Bachelors or Masters degree in Occupational Therapy from an accredited program
Valid Occupational Therapist Registered (OTR) license in Pennsylvania (PA)
Minimum of 1 year of healthcare experience
Current AHA BLS certification (ACLS or PALS accepted)
Valid drivers license and reliable transportation
Experience with EMR systems and mobile technology
Strong self-management time management and documentation skills
