Job Summary:
This position is for a Home Health Occupational Therapist in West Portland OR and surrounding areas. The therapist will provide occupational therapy services to patients in their homes focusing on maximizing their functional independence. This role requires a current Oregon license BCLS certification and at least one year of skilled home health experience.

Responsibilities:
- Provide occupational therapy services to patients in their homes.
- Assess patient needs and develop individualized treatment plans.
- Implement therapeutic interventions to improve patients functional abilities.
- Educate patients and their families on therapy techniques and home exercise programs.
- Document patient progress and communicate with the healthcare team.
- Maintain accurate and timely records using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/ Oasis.
- Travel to patient homes within a roughly 60-mile radius.
Required Skills & Certifications:
- Active Oregon Occupational Therapist license.
- BCLS certification.
- Minimum 1 year of skilled home health experience.
- Oasis experience.
- Primary series COVID vaccination in addition to one booster.
- Valid drivers license and reliable transportation.
Preferred Skills & Certifications:

Special Considerations:
- Traveler is expected to have their own transportation.
- Facility can call off UP TO one shift per pay period (every two weeks).
- The facility requires primary series COVID vaccination in addition to one booster.
- Local candidates within a 50-75 mile radius are accepted at the same rate.
- Traveler is expected to have their own transportation.
Scheduling:
- 8-hour day shifts.
- The facility cannot accommodate 4x10s or 3x12s.
- No weekend requirement.
- No on-call requirement.
- Block schedules are not accommodated.
