Our client is seeking a dedicated Occupational Therapist to join their home health care team in Somerset PA. The position focuses on providing high-quality in-home therapy services improving patients independence and supporting overall wellness in a collaborative team-based environment. This role includes eligibility for a $10000 first-year success bonus based on key performance milestones.
Key Responsibilities
Conduct occupational therapy assessments to determine patient functional levels and establish individualized care plans
Implement and adjust treatment programs approved by the attending physician
Educate patients and family members on exercises adaptive equipment and safety practices
Design and train patients in the use of customized adaptive or assistive devices
Maintain accurate clinical and progress documentation within required timeframes
Communicate with physicians nurses and interdisciplinary care teams regarding progress and outcomes
Support care coordination and participate in team meetings case reviews and staff development
Schedule and Shift Details
Monday through Friday 8:00am to 5:00pm
Full-time no weekends holidays or on-call requirements
Travel
Local travel within Somerset and surrounding areas
Required Qualifications
Bachelors or Masters degree in Occupational Therapy from an accredited institution
Valid Occupational Therapist Registered (OTR) license in Pennsylvania (PA)
Minimum of one year of healthcare or occupational therapy experience
Current drivers license and reliable transportation
Proficiency with EMR systems tablets and mobile technology
Ability to manage scheduling travel and documentation independently
Preferred
OT license number provided upon submission
