Overview
The Speech Therapist provides in-home speech therapy services to patients as prescribed by the attending physician. This role includes evaluating planning and implementing rehabilitation services for speech language and swallowing disorders. The therapist will conduct diagnostic assessments establish goals document progress and collaborate with patients families and healthcare team members to support communication and swallowing improvement.
Key Responsibilities
Assess plan and implement individualized rehabilitation programs for speech language and swallowing disorders
Conduct diagnostic testing to evaluate speech and language abilities establishing goals and treatment plans based on findings
Monitor and document patient progress adjusting treatment plans as necessary
Communicate changes in patient condition and progress to the physician and care team
Educate family members and caregivers on strategies to support communication and swallowing function at home
Participate in case conferences in-service trainings and staff meetings
Prepare and submit clinical and progress notes reports and documentation in accordance with policy and regulatory requirements
Maintain professional development and clinical competency through continuing education and in-service programs
Perform other related duties as assigned
Schedule and Shift Details
Full-time Monday through Friday 8 AM 5 PM (40 hours/week; 18 30 units/week)
No weekends holidays or on-call requirements
Travel
Local travel required; employee will commute by automobile and may be exposed to varying weather conditions
Qualifications Required
Masters degree or higher in Speech Language Pathology
Minimum 1 year of experience in Home Health or a related healthcare setting
Valid Speech Language Pathologist (SLP) license in Pennsylvania (PA)
Current Basic Life Support (BLS) certification (AHA)
Valid drivers license and reliable transportation
Preferred
Experience with electronic medical record systems (e.g. Epic Home Health)
One year of supervised post-certification experience in an institutional setting
