Job Description
A well-established home health and hospice organization in New Hampshire is seeking a compassionate Home Health RN to support patients in the Keene area. This is an excellent opportunity to build meaningful relationships provide individualized care and make a true impact in patients lives all outside a traditional clinical setting.
What Youll Do
- Deliver one-on-one nursing care to patients in their homes.
- Complete thorough initial assessments to identify patient needs.
- Develop and adjust personalized plans of care.
- Administer medications and treatments as ordered.
- Educate and support patients and their families throughout the care process.
- Document all care provided and track progress toward established goals.
What Were Looking For
- Graduate of an accredited nursing program.
- Active RN license.
- Minimum of 1 year of nursing experience.
Position Details
- Full-time permanent role
- 30-40 hours/week day shift
- Employees who work 30 hours/week are eligible for full-time benefits
