Reports To: SVP Community based Services

multiple states providing K-12 alternative education comprehensive mental and behavioral health
services coaching counseling and trauma-informed training.
Additionally TLC collaborates with school districts universities hospitals police departments
and nonprofit organizations offering consultation services to secure federal and state grant
funding
Position Summary: The Interventionist will provide general case management substance abuse
counseling The Interventionist is responsible for establishing a professional and supportive first
contact for intake will conduct initial screenings for services and provide ongoing support.
Duties & Responsibilities
Case management:
Enter all documentation of direct and indirect services into Simple Practice within 5 business
days from the service.
Conduct initial intake and needs assessment interview with all clients.
Provide therapeutic interventions; specifically CBT modality and develop treatment plan for
inmates.
Monitor clinical needs of inmates in caseload and conduct small group sessions.
Maintain accurate documentation of service objectives and outcomes as well as other services in
accordance with guidelines.
Actively participate in staff meetings and trainings.
Prepare related reports
Adhere to strict boundaries and professional ethics in the care of others.
Conduct crisis intervention as needed
Educate on treatment options methods and techniques to ensure the patient understands the
process
Work with other professional staff to collaborate on treatment methods.
Comply with all company policies and procedures as outlined in the employee handbook

Bilingual a plus
Education & Experience:
Minimum of Bachelor degree in human service field (psychology counseling social work
etc.).
Masters degree and/or License Social Work (LSW) preferred.
One or more years of experience in a Mental Health treatment program preferably in a
Correctional Facility.

Work Environment: Vermont Department of Corrections facility
Work Hours: This is a full-time 40 hours per week position. Evening and/or weekend work can be
expected as well as the sudden need for crisis intervention.
Travel: Use of a reliable car is necessary for travel to service sites.
