We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our clients growing team in Bushkill Pennsylvania. This role focuses on delivering skilled patient-centered care in the comfort of patients homes helping them maintain independence and improve overall well-being. The ideal candidate will bring clinical expertise strong communication skills and a genuine commitment to providing high-quality care.

Key Responsibilities
Deliver professional nursing care and perform comprehensive patient assessments in home settings
Develop individualized care plans in collaboration with physicians and the healthcare team
Educate patients and families on treatment plans medications and post-care instructions
Monitor patient progress and adjust care as needed to achieve optimal outcomes
Maintain accurate and timely documentation in accordance with regulatory standards
Ensure effective communication and coordination among care team members

Schedule and Shift Details
Monday through Friday 8:00am 5:00pm
On-call weekend rotation every 4 8 weeks (Saturday and Sunday 8:00am 5:00pm)
Flexible scheduling with minimal weekend work
This is a Pay Per Visit position

Travel
Local travel required within the Bushkill area (travel reimbursement provided)

Qualifications
Required
Associate degree or higher in Nursing
Active Registered Nurse (RN) license in Pennsylvania
Minimum of 1 year of professional RN experience (home health preferred)
Current BLS or ACLS certification through the American Heart Association
Valid drivers license reliable vehicle and current auto insurance

Preferred
Prior experience in home health or community-based nursing
Strong assessment communication and time management skills
