Job Summary

Saint Alphonsus Regional Medical Center is looking for a Hospitalist OB that will cover the dates for both Boise and Nampa locations!

Schedule:
Night float (Sun-Thurs 7p-7a) in-house every other month
Fri-Sun (7a-7p) whole weekend in-house not required
Dates Needed Coverage (Boise & Nampa): candidate must have this availability for each month needed:
March - 8 shifts
April 9 - shifts
May - 10 shifts
June - 10 shifts
Travel Requirements:
Flights - $750/round trip cap (to include all taxes and fees)
Lodging - $179/night cap for hotels (to include all taxes and fees). Preferred Lodging: Candlewood Suites Boise Towne Square an IHG Hotel.
Rental Car - To be reimbursed by facility (not to be used with personal car/mileage reimbursement). $50/day cap (including all taxes and fees) regardless of car class. SUV/upgrades over cap will need to be preapproved (will only be approved if needed for inclement weather).
Mileage - IRS Standard Rate to be reimbursed by facility (only to be used with personal vehicle not to be used with rental car).
Bill Rates Needed for Submission:
Malpractice (must be included in the rates)
Orientation
Hourly ($295 rate cap)
Holiday Hourly
Overtime
Holiday Overtime
Conversion fee ($25k rate cap)

MD diploma
Internship/residency/fellowship certs
All medical licenses- COVID-Related Physician Licensure Exemption - Idaho Code 54-1804(1)(d)
DEA
Board Certification
Life support cards- BLS
Current and previous certificates of insurance
Driver s license
Professional color photo/headshot
CMEs for the past 2 years
Immunization records including TB Flu HepB Tdap Varicella and COVID
References
Case logs
