Location: Hervey Bay MonFri 8:00am 4:30pm

Just Better Care is seeking a dedicated Clinical Care Partner to join our Hervey Bay this community-based role youll provide personalised support to older Australians helping them live safely and independently at home.

Key Responsibilities

Complete in-home assessments and care planning
Manage participant cases and provide clinical/holistic support
Track participant wellbeing and document progress and goals
Liaise with medical professionals allied health and service providers
Maintain documentation to ensure compliance (CRM funding evidence uploads)
Oversee participant budgets and ensure funding guidelines are met
Support training and guidance of support workers and nurses
Schedule and coordinate services including urgent changes
Contribute to ongoing audits reporting and professional development
(Clinical Care Partners only) Provide occasional wound care

What Youll Need

Advanced computer & CRM skills
Strong administration organisation and multitasking abilities
Excellent communication and customer service skills
Ability to work both independently and in a team
Knowledge of wellness reablement and enablement approaches
Current First Aid/CPR
Valid Police Check NDIS Worker Screening Card & WWCC
Current Australian Drivers Licence

Desirable

Experience in home/community/aged care
Knowledge of Home Care Package guidelines & compliance
Current AHPRA registration (RN or EN)
