The Mass General Brigham Medical Group is a system-led operating entity formed by Mass General Brigham to deliver high quality low cost innovative community-based ambulatory care. The Medical Group provides a wide range of offerings including primary care specialty care behavioral and mental health and urgent care both digitally as well as at physical locations in Massachusetts New Hampshire and Maine. Located at 600 Primrose Street in Haverhill MA our Mass General Brigham practice offers comprehensive and compassionate care across several specialties including Internal Medicine Infectious Disease and our Bridge Clinic. Our Internal Medicine team provides primary and preventive care for adult patients managing both acute and chronic conditions. The Infectious Disease specialists offer expert evaluation and treatment for a wide range of infections with a focus on evidence-based care and patient education. The Bridge Clinic provides integrated patient-centered support for individuals with substance use disorders helping patients access treatment recovery resources and ongoing medical care in a welcoming and nonjudgmental environment. Together these services reflect our commitment to accessible high-quality care for every patient we serve.

We are seeking a full-time 40-hr Registered Nurse to join our Infectious Disease practice! The desired schedule is Monday through Friday from 8:30am to 5:00pm located at 600 Primrose Street in Haverhill Ma.


Job Summary

The Infectious Disease Nurse Case Manager provides comprehensive clinical care and case management services to patients with complex infectious disease needs. This includes patients with hepatitis B or C individuals living with HIV those receiving Pre-Exposure Prophylaxis (PrEP) and patients requiring outpatient parenteral antimicrobial therapy (OPAT) among others.

Key responsibilities include:
Delivering direct patient care including medication administration and patient education.
Coordinating care across internal teams and community resources.
Supporting patient adherence and facilitating smooth transitions of care.
Monitoring outcomes to ensure the highest quality of services.
In addition this role will provide support to Adult Medicine/Primary Care patients as needed serving as a bridge during coverage gaps. This role also provides support to patients in our Bridge Clinic providing substance use services during coverage gaps.

Education
Associates Degree Nursing required or Bachelors Degree Nursing preferred

Licenses and Credentials
Registered Nurse RN - State License - Generic - HR Only required Basic Life Support BLS Certification - Data Conversion - Various Issuers preferred

Experience
Case management utilization review or discharge planning experience 2-3 years preferred.

Knowledge Skills and Abilities

  • Administer vaccines (Subcutaneous and Intramuscular) and injectable medications (Intramuscular).
  • Provide treatment support and education for patients with HIV and those initiating or maintaining PrEP.
  • Deliver basic wound care management for infectious disease patients as clinically indicated.
  • Oversee coordination of intravenous (IV) antibiotic services including patient education and monitoring.
  • Manage OPAT services ensuring seamless communication between hospital outpatient clinics home health services and infusion centers.
  • Coordinate medication delivery for patients ensuring access timely delivery and adherence support.
  • Serve as a liaison between patients providers pharmacies and community-based resources to support holistic care.
  • Communicates with third party payers to obtain necessary authorization for reimbursement of services.

    Proficient in Microsoft Office and industry related software programs.
  • Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
  • Ability to maintain client and staff confidentiality.
  • Ability to provide stigma free care to patients.
  • Understanding of diagnostic criteria for dual conditions and the ability to conceptualize modalities and placement criteria within the continuum of care.
  • Knowledge of Healthcare and Managed Care preferred.
