- Provide leadership in advancing professional nursing and interprofessional practice across the organization
- Support staff and leaders in interpreting and applying standards of practice scope of practice and ethical decision-making
- Collaborate with clinical leaders to develop and implement policies procedures and clinical guidelines that reflect evidence-based practice.
- Champion a culture of professional excellence clinical accountability and reflective practice
- Lead the planning implementation and evaluation of the organizations BPSO designation in partnership with RNAO
- Facilitate the selection implementation and sustainability of RNAO Best Practice Guidelines (BPGs) across homecare teams
- Coordinate BPSO committees champions and workgroups to promote engagement and adoption of best practices
- Prepare and submit required progress reports outcome data and success stories to RNAO and internal stakeholders
- Develop strategies to scale and spread best practice innovations across the organization
- Develop and deliver educational programs and workshops to strengthen clinical competence professional judgment and best practice implementation.
- Mentor clinical educators nurses and interprofessional staff in evidence-based practice and change management
- Support orientation and ongoing professional development programs to align with BPSO and professional practice priorities
- Partner with the Quality and Risk teams to embed best practices into quality improvement patient safety and outcome measurement frameworks.
- Analyze data to identify trends measure the impact of practice changes and inform continuous improvement
- Contribute to the development of quality indicators related to clinical practice nursing excellence and client outcomes
- Collaborate with internal departments regional partners Ontario Health at Home and external stakeholders to align practice standards
- Act as a professional role model and advocate for homecare nursing and interprofessional practice
- Represent the organization at external forums professional networks and BPSO learning events.
Qualifications :
- Education: Bachelors degree in Nursing (BScN) required; Masters degree in Nursing Education or Health Administration preferred
- Current Certificate of Registration in good standing with the College of Nurses of Ontario (CNO)
- Experience: Minimum of 5 years of progressive clinical leadership experience in community or homecare settings
- Demonstrated experience with RNAO BPSO implementation or evidence-based practice initiatives
- Proven success in leading professional practice programs education or quality improvement projects
- In-depth understanding of nursing and interprofessional practice standards quality improvement and change management
- Strong knowledge of RNAO Best Practice Guidelines and the BPSO framework
- Excellent facilitation coaching and communication skills
- Ability to analyze data interpret outcomes and translate evidence into practice.
- Travel to homecare branches as required
- May require flexible hours to support clinical operations and staff education
