Overview
We are seeking an experienced and compassionate Home Health Admission Registered Nurse (RN) to join our home health care team in the Greensburg and Irwin Pennsylvania areas. Key Responsibilities

• Perform comprehensive Start of Care (SOC) assessments for new home health patients

• Develop and manage individualized care plans tailored to each patients needs

• Deliver direct nursing care in accordance with physician orders and care standards

• Collaborate with interdisciplinary team members to ensure high-quality outcomes

• Maintain accurate and timely documentation using EMR systems and mobile technology

• Manage daily schedule and prioritize patient visits efficiently

• Educate patients and families about care plans medications and follow-up needs

Schedule and Shift Details

• Schedule: Monday Friday 8-hour shifts

• On-call: One evening per week (5:00 p.m. 10:00 p.m.) and one weekend per month (Saturday and Sunday)

Qualifications

Required

• Associate degree in Nursing or higher

• Current Registered Nurse (RN) license in the state of Pennsylvania

• Valid Drivers License and reliable transportation

• Minimum of 1 year of experience with electronic medical record (EMR) systems

• Current Basic Life Support (BLS) certification from the American Heart Association or equivalent

Preferred

• Advanced Cardiac Life Support (ACLS) certification

• Experience in home health admissions community nursing or skilled home care
