Key Responsibilities

Deliver home-based physical therapy services to patients across Benton Harbor and nearby areas
Perform comprehensive assessments and evaluations to determine patient needs and treatment goals
Develop implement and regularly update individualized care plans
Provide therapeutic interventions to improve mobility strength and functional independence
Collaborate with interdisciplinary team members to ensure quality coordinated care
Educate patients and caregivers on therapeutic exercises and recovery strategies
Maintain accurate timely documentation in the EMR system
Ensure compliance with clinical standards policies and regulations

Schedule and Shift Details

Full-time 40 hours per week

Qualifications

Required

Bachelors or Masters degree in Physical Therapy from an accredited institution
Active Physical Therapist license in the state of Michigan
Minimum of 1 year of healthcare experience preferably in home health
Valid drivers license reliable transportation and ability to travel (approximately 20% of the day)
Experience with Electronic Medical Record (EMR) systems
Strong time management and self-scheduling skills
Proficiency with mobile technology and documentation tools

Preferred

Previous home health care experience
PT license number provided upon application

Additional Information

Successful completion of a pre-employment drug screen required (includes medical and recreational marijuana)
Must be legally authorized to work in the United States
