The Role
As a Clinical Case Manager you will oversee and coordinate the delivery of holistic person-centred care to clients living in their own homes. Youll use your clinical knowledge and leadership to assess needs plan care and support clients to maintain independence and wellbeing.
Key Responsibilities
- Conduct comprehensive nursing assessments and develop tailored care plans
- Provide clinical oversight and support to care staff and allied health partners
- Monitor care outcomes and adjust plans to meet changing needs
- Build strong trusting relationships with clients and their families
- Maintain accurate clinical documentation and ensure compliance with regulatory standards
- Collaborate with multidisciplinary teams to achieve the best health outcomes
Requirements
- Current AHPRA registration as a Registered Nurse
- Demonstrated experience in community or aged care settings
- Excellent communication and time management skills
- Ability to think critically and respond effectively in dynamic situations
- Current drivers licence and reliable insured vehicle
- National Police Check and NDIS Worker Screening Check (or ability to obtain)
- Full Australian working rights
