Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other team members are essential. The position may require floating between units.

Responsibilities:
- Provide skilled nursing care
- Perform patient assessments
- Administer medications
- Develop and implement treatment plans
- Collaborate with the interdisciplinary team
- Communicate effectively with patients providers and team members
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of working
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Neuro experience
Special Considerations:
- May float between units
- Weekends are required (every other weekend)
- First-time travelers are welcome
Scheduling:
- 8-hour shifts
- Weekends required (every other weekend)
