Job Summary

Social Worker Specialist Coordinator Aged Care

As a P3 Social Work Aged Care Specialist Co-ordinator position you will oversee the co-ordination at Joondalup Health Campus for services to aged care patients and the management of the ACA program with linkages to both community and residential services in the aged care field.

Collaborates and liaises with Government Departments at State and Commonwealth levels to ensure that policy and procedures meet best practice initiatives.

Applicants must address the criteria against the Job Description to be considered.

1. Bachelor of Social Work or approved equivalent.

2. Eligible for membership to the Australian Association of Social Workers. Clinical Practice

3. Demonstrated high level of expertise in required area of clinical practice (Aged Care)

4. Sound knowledge of legislation pertaining to Aged Care including working knowledge of services provided through My Aged Care

5. Current ACA assessor : Desirable ACA Delegate

6. Advanced knowledge of community and residential aged care services.

7. Proven ability to plan initiate and report on Key Performance Indicators quality improvement activities and evaluate the effectiveness of service provision in allocated area.

8. Proven ability to collect analyses interpret and present clinical data including use of appropriate statistical tools and methods.
