Job Summary

Across our 60 locations we provide programs in a variety of service areas including workforce development children youth families seniors and homeless prevention.

Our family shelters are safe welcoming spaces where families can stay temporarily while they work toward long-term stability. Program staff provide compassionate support life-enhancing services and community-based resources to help empower each family. Together we guide families on their journey back to independence and into permanent housing.


RESPONSIBILITIES

  • Provide trauma-informed voluntary case management services to clients and their families to meet the overarching program goal of maintaining permanent housing and achieving a higher level of independence.
  • Manage a caseload of approximately 40-50 families three to six months being the average length of an open case.
  • Conduct 50 face-to-face client contacts each month which will be a combination of clients on site in the last month of their shelter stay and those out in the field who moved out into permanent housing.
  • Develop review and adjust service plan collaboratively with the client covering the programs primary service areas: finances and housing education employment health and mental health and legal (as needed). Work with clients to break down necessary tasks and strategies to achieve the goals outlined in the service plan. After six months of service delivery review the service plan to assess a clients progress toward independence.
  • Ensure that clients have maximized access to public benefits and maintain an active public assistance case. This includes understanding the various New York City housing voucher eligibility and re-certification requirements.
  • Ensure education remains a focus area of each case. Aid families in ensuring children are enrolled timely and are consistently attending school following potential disruptions resulting from moving. Identify and work through barriers to attendance and education.
  • Communicate with collateral contacts on clients behalf. Build positive working relationships with service providers such as HRA ACS legal aid childcare and education professionals. Submit referrals to additional service providers adherent to client goals.
  • Connect clients and their families with events and resources within their new neighborhoods to boost community engagement. This may include workshops or training relevant to their education and employment goals.
  • Collaborate with BronxWorks Family Shelter social services staff to identify and recruit potential program participants.
  • Actively participate in bi-weekly supervision with your direct supervisor to support successful case resolution manage complex client needs and maximize efficiency in program delivery.
  • Actively seek out and participate in professional development training to continuously improve service delivery to clients and their families.
  • Document all interactions with or on behalf of clients. Monitor and document client progress toward service plan goals.
  • Contribute ideas for enhancing and expanding services provided to clients and families through the Transform Program.
  • Perform additional duties as assigned which may include but are not limited to administrative programmatic tasks.

QUALIFICATIONS

  • Associates degree in a related field required Bachelors preferred.
  • Minimum of one year of experience in a related field.
  • Proficiency in modern business communications including personal computers cell phones electronic mail voicemail facsimile and copier equipment.
  • Strong preference will be given to candidates who are proficient in Spanish or French.

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand walk or sit for long periods of time.
  • Ability to climb five flights of stairs if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather including inclement weather if required to conduct home visits and/or field work.
